Exception lists swell when every incompleteness is labeled a control break. Aging then becomes a moral drama instead of a work queue. Module 5 of our flagship program spends more time on naming than on software clicks for that reason.

Three tags that reduce noise

Evidence gap. Attachment missing, rationale blank, export truncated. Fixable without redesigning the control.

Design gap. The app cannot enforce what policy requires (for example, mobile skips a field). Needs a compensating check or a ticket — not an endless “pending.”

Control break. Approval outside authority, dual control skipped, or deliberate override without recorded reason.

Owners and clocks

Every open item needs a named human and a date that means something. “Finance” is not an owner. “Next week” is not a clock. When retests pass, write the retest note on the same row — do not start a parallel spreadsheet that drifts.

If your financial auditing app for transaction approval audits already has exception objects, use them. If it does not, a shared table with those three tags still beats a free-text chat dump.
